How to Prepare

Arrive 10–15 minutes early to complete check-in and confirm your information.

Bring a photo ID, your insurance card(s), and a list of current medications and allergies.

Bring any prior records or imaging if available. Athletes — bring your most-used shoes or orthotics.

For new injuries, note when symptoms started, what makes them better or worse, and any treatments you've already tried.

Patients under 18 must be accompanied by a parent or legal guardian.
Insurance & Billing

Our team verifies benefits and files claims on your behalf. Copays, coinsurance, and unmet deductibles are due at time of service. If your plan requires a referral or authorization, please obtain it before your appointment.
